Collaboration between MCPs and CBOs is vital for the success of the CalAIM program. However, the initiative faces challenges stemming from structural and cultural mismatches and a lack of effective technology tools. MCPs are focused on expanding CBO networks, providing training resources, ensuring compliance, and improving cost efficiency and auditing processes. Meanwhile, CBOs are struggling to keep pace with MCPs’ evolving reporting and data-sharing demands while continuing to deliver personalized, community-based care. While provider training sessions and office hours have helped build trust, collaboration outside these interactions is often restricted to monthly MIF/RTF exchanges—reactive processes centered on data merging with minimal actionable outcomes at the population level. This misalignment causes delays, missed opportunities, and difficulty in aligning on shared goals.

Closed-loop technology that enable providers to coordinate on referrals and notify stakeholders of critical updates can ensure timely action and further minimize duplication.
